Corrective Action and Follow-up Report
Corrective Action and Follow-up Report
A corrective-action report demonstrates how a finding was controlled, investigated, addressed and checked for effectiveness. Closing an action requires evidence, not only a statement that work is complete.
## 1. Corrective-action sequence 1. Record the finding and objective evidence. 2. Apply immediate correction or containment where necessary. 3. Assess actual and potential environmental consequences. 4. Investigate the root cause using an appropriate method. 5. Define corrective actions that address the cause. 6. Assign an owner, resources and target date. 7. Verify implementation with objective evidence. 8. Check effectiveness after a suitable operating period. 9. Close or reopen the action with documented approval.
## 2. Action register template | Field | Required entry | |—|—| | Action ID | CA-YYYY-001 | | Related finding | Inspection / monitoring / audit reference | | Immediate correction | Containment or correction taken | | Root cause | Evidence-based cause, not a symptom | | Corrective action | Specific action preventing recurrence | | Owner | Responsible role | | Due date | Approved target | | Completion evidence | Records, photos, measurements or revised procedure | | Effectiveness criterion | Measurable evidence that recurrence is controlled | | Status | Open / In progress / Pending verification / Closed |
## 3. Root-cause analysis Use a proportionate method such as 5 Whys, cause-and-effect analysis or process review. Avoid unsupported conclusions such as “operator error” without investigating training, procedure design, supervision, equipment condition and workload.
## 4. Effectiveness verification Define the verification method before closure. Examples include repeat sampling, inspection after a defined operating period, review of maintenance records or evidence that a revised control is consistently implemented.
## 5. Escalation and overdue actions Identify overdue or ineffective actions, environmental risks requiring interim controls, responsible management and any external notification obligations. Escalation timelines must follow the applicable permit and procedure.
