## Minimum documentation principles - Every report has a unique reference, version, date and approval record. - Every finding or result links to traceable evidence and a defined method. - Units, criteria, averaging periods and reporting limits are stated. - Limitations, missing data, deviations and uncertainty are disclosed. - Conclusions do not extend beyond the evidence and assessment scope. - Recommendations identify responsibility, timing and verification where applicable.
Important: These are general technical frameworks, not official government forms. Use the competent authority’s prescribed format and the applicable permit, approved method and jurisdictional requirements. Example rows are placeholders, not actual site or laboratory results.
